SERVICE AGREEMENT INFORMATION

Steps with the service agreement:

  • Our practice manager will develop a service agreement, including the anticipated costs and steps required to support you / your client to work towards their goal.
  • The service agreement needs to be reviewed by the Participant and signed, confirming that there are sufficient funds in the “Capacity Building: improved daily living” budget.
  • If applicable, we will send the signed service agreement to the plan manager and support coordinator, to confirm that there is sufficient funding available.

Running out of Funding:

We are a small business with quality staff and a lot of operating costs – we cannot run our business if invoices are not paid because you have “run out of funding”. We don’t get funding from anywhere else.

Consider this:

  • would you go to the doctor and not pay your bill?
  • would you go to the hairdresser and not pay your bill?
  • would you go to Woolworths and not pay for your groceries?

If you / your client signs the service agreement confirming sufficient funds, but these are not available, then it will be the client’s responsibility to pay the invoice.

We will issue invoices after each part of the service is delivered (that is – after your initial assessment, after your report, after any further consultations, stages of your funding application).

We will not be able to proceed with the next stages of your service (including commencing and release your report and/or application), until all invoices are paid in full.

CANCELLATION POLICY
We understand that driving and vehicle modifications assessments are a complex service requiring significant preparation from both you and Driving Well OT. We value the time and effort you’ve invested in this process and want to ensure that everything goes smoothly.

Steps Taken for Your Appointment:

  • Coordinating schedules among you, your family, the occupational therapist, the equipment supplier, to find the perfect appointment slot.
  • Sending all necessary documents to Driving Well
  • Signing your service agreement with Driving Well.

We appreciate your dedication and the time spent preparing for your appointment.

Appointment Commitment:
As you know, each part of the assessment takes between ~2 and 3 hours. If you are unable to keep your appointment, it is extremely difficult to fill that slot with another client on short notice. Therefore, we need ample notice to reschedule assessments effectively.

Important Requirements:

  • All paperwork must be received at least seven days prior to your appointment. Failure to do so will result in the postponement of your appointment.
  • We require at least seven days’ notice for any cancellation
  • Cancellations within two business days will incur a 100% fee for the face-to-face appointment.
  • Please understand that we need to assure commitment to your appointment. Cancellations made between three and five business days without a valid reason (such as hospitalization of yourself or an immediate family member) may result in Driving Well being unable to continue working with you.
Fee Schedules for self-funded clients:
